Social Navigator (0.8 FTE)

The Opportunity

We are seeking an individual who is passionate about promoting health and wellness equity in the DTES to assist the Social Navigation program at Kilala Lelum in the role of Social Navigator (our reimagined version of a Social Worker role). 

Job Summary

The Social Navigator (0.8 FTE), working within an interdisciplinary care team that includes Indigenous Elders, Physicians, Nurses, Allied Health professionals, and Outreach Workers, delivers a plan of care and provides direct care activities to members within the Drop-in Program and through internal referrals. The Drop-in Program is designed to provide immediate and accessible support to members, particularly those facing urgent or acute challenges.

Compensation

  • $39.53 per hour at 32 hours per week
  • Extended Health Benefits and Dental available after successful completion of 3 month probationary period
  • RRSP matching up to 5% available after successful completion of 3 month probationary period

Roles and Responsibilities

  • Support members who are adversely affected by social determinants of health and who experience health challenges including but not limited to: chronic illness, substance use, and mental health and wellbeing;

  • Attend weekly Social Navigation Team meetings, and monthly Allied Wellness meetings;

  • Facilitate Social Navigation Drop-In as scheduled amongst Social Navigation team;

  • Support with service navigation across health, government, and non-profit services;

  • Provide resources and referrals via inreach and outreach services to support members, as required;

  • Coordinate a plan of care and support transitions between services; demonstrate a comprehensive knowledge of other health disciplines and their roles in care;

  • Provide immediate psycho-social support to members utilizing the Drop-in Program, with a focus on acute needs, and crisis intervention and prevention;

  • Establish, maintain, and enhance therapeutic relationships based on respect, cultural safety, trauma-informed practice, and harm reduction principles;

  • Perform emergent or time-sensitive care as required, including occasional appointment accompaniments;

  • Document work in OSCAR Electronic Medical Records (EMR) and Lu’Ma Coordinated Access Homelessness Management Information System (HMIS). 

Qualifications & Skills

  • Bachelor of Social Work (BSW) required with Master of Social Work (MSW) strongly preferred. At least two (2) years of recent relevant work experience, with strong preference for experience working in Vancouver’s DTES providing care through a holistic mental health, substance use, and primary care lens. An equivalent combination of education, training, and experience will be considered;

  • Current full registration with the British Columbia College of Social Workers, or a willingness to obtain registration;

  • Candidates with Indigenous ancestry encouraged to apply;

  • Ability to work in a fast-paced, drop-in environment with diverse community members;

  • Experience with crisis intervention and acute care management strongly preferred;

  • Effective communicator, both verbally and in writing;

  • Demonstrated understanding, commitment, and ability to be culturally safe and culturally appropriate with patients;

  • Demonstrated commitment to trauma-informed, member-centered, harm reductive and recovery-oriented practices and processes;

  • Experience and knowledge of Indigenous Peoples and Indigenous Cultures is considered an asset; and,

  • Mandatory completion of a cultural competency course through orientation process.

Supervision Provided

This position is supervised by the Allied Health Lead, and will receive support from the Social Navigation Manager and members of the Allied Health team, including other Social Navigators.

  • The position time will be split between Doris Fox Lelum (121 Heatley) and the Primary Care clinic (626 Powell Street)

  • Successful candidates will be required to complete a Criminal Record Check
